CES utilizes an account application which may be found here on our Web site or by requesting a copy from CES sales or admin personnel. The form can be faxed or emailed to your attention. In addition, you may submit your financial information in your format and attach it to our Account Application form. After a quick review of the application, an account may be established for your company. In lieu of the account application, a valid MasterCard or Visa may be utilized for payment in advance.
